2 / Non-restrictive description of risks: By considering my participation or that of my child in this activity, I hereby acknowledge that I have been informed of the risks and dangers, including but not limited to those associated or related to the practice of soccer. And I acknowledge and agree to the risks that I or my child may, as a participant, suffer a physical injury or illness (minor, serious, catastrophic and / or fatal). In addition, I am informed and warned that injuries sustained while playing soccer can make my child disabled, or even permanently paralyzed.

3 / Consent to medical care and discharge: In the event of injury or illness, I authorize CogiFit, as well as its educators, to obtain the medical treatments required by my child and I release and do not hold responsible the executors of this authority. I also understand and agree to the fact that I am responsible for paying the costs and invoices related to the injury or illness suffered during the participation of my child in the activity.

4 / Discipline: The child must respect all the other children as well as the educators of CogiFit in his language, his gestures and his attitudes. In order to maintain a high standard of quality during training sessions, CogiFit reserves the right to notify the family of a child adversely affecting the development of other participants of the end of the participation of the young person in question in the chosen program. Administrative costs, a pro rata relative to the practices carried out as well as the products and services made available for the practices will thus be taken into account for the reimbursement.

5 / Image rights: I authorize CogiFit to use the photographs and / or videos taken of me and / or my child (ren), to print them, publish them, distribute them or use them for CogiFit communications tools (publications, website, social media, electronic signs, etc.) without remuneration.
